All participants are required to provide a certified copy of a birth certificate in order to be enrolled in any program we offer.  Once we have obtained a copy, we will keep it on file for future registrations.  We also require that you provide proof of residency which would include any of the following:

• Water Bill
• Telephone Bill
• Utility Bill

A birth certificate is not considered proof of residency.  Thank you for your cooperation.
